I don't think it's even the picture quality, but X-Rated Kult's Women Behind Bars (aka Frauengefängnis 2 aka Jailhouse Wardress aka Gardiennes du Penitencier) is an awful movie. It's pieced together from at least three different movies - Frauengefängnis (pt 1?), Fraulein Devil and Special Train for Hitler. And it wasn't a great patchwork. It's cropped to 1.33:1 from whatever the aspect ratio of the donor films were.

Since the question was about "worst that we'd seen," the cover of the same company's release of D'Amato's Death Smiles on the Murderer looks like a picture taken from a cheap children's storybook that got wet or something.

Telefon, 70s Bronson movie. The DVD is shite of the highest. Full screen apparently ripped from a VHS copy, complete with fuzzy picture and crackling sound. WTF.

Absolutely the worst Blu ray picture I have seen. Disc has been boosted so much that any light scenes, or light clothes are full of speckles.
Picture looks worse than a bad VHS.
The Blu ray is part of a box set but you can pick it up on it's own.
That's what I did, and it was not cheap, but now I am stuck with this worthless pile of s**t.

Stick with the DVD.
