Unfortunately Exterminator 2 did not fair well at the the box office otherwise there might have been a 3rd film.

Yes when you watch these films years later they often look quite tame.

Exterminator 2 had some footage removed by the MPAA. We got the R. I think the unrated version is probably lost forever.

It was the R rated version that was submitted to the BBFC in 1980. As far as I know the R version was uncut, the cuts listed above appear to be the cuts the BBFC made not the MPAA.

The video version of The Exterminator classified by the BBFC in the 80's was the one with the additional BBFC cuts (on top of the cinema cuts). I don't think it was the R version. Did the MPAA ever...
