Rob van Dam botches a five-star frog splash during the first Elimination chamber match, held at the Survivor Series PPV in November 2002


He misjudged his leap from the top of the pod, and when he landed on HHH, his shin was across The Game's throat, resulting in a legitimately injury

In a feature from ESPN in 2017:

"RVD hopped up to the top rope to set up for a five-star frog splash, but then he turned around and instead hopped up on top of one of the pods. He crouched on the edge of the pod, grabbing the chain above him for support and did his best to stand up -- but he couldn't.

Jericho, 2017: The pods weren't high enough to really stand on, but they were high enough for you to try and stand on it. It was a disaster.

Van Dam, 2002: I jumped out a little too far and I crunched up into a ball, and my instincts told me to straighten my legs out -- and somehow, I think my shin came across his throat with the weight.

Triple H writhed around in pain, kicking his legs over and over again on the mat. It didn't take long to figure out he's not just selling the move.

Triple H: Van Dam lands on my throat -- completely crazy, random moment in time. He was at an awkward angle, and he couldn't stand up straight.

Van Dam, 2002: I had to alter my five-star frog splash because of the structure. When I looked down on him, I thought everything was going to be fine. I mean, I knew he was in a little bit close, but I thought that I'd just be able to drop right down on him.

Triple H: It seemed good when we put it together. He landed on my throat, and I couldn't breathe and I couldn't speak.

[Referee Earl] Hebner bent down to check on Triple H, and appeared to subtly signal to the back that something had gone very wrong while a replay of the moment aired on a split screen. After the replay finished, Hebner stood over Triple H and put his arms up in a deliberate X to again signal the injury.

Triple H: They sent a doctor into the ring, who was talking to me through the grates, and I was like "Do not stop this, I got it." But I couldn't talk."

Another Elimination Chamber botch

During his entrance at the 2010 Elimination Chamber PPV, The Undertaker is caught up in a mistimed pyro burst, resulting in him suffering from 1st and 2nd degree burns

- John Morrison defeated Mustafa Ali, Ricochet, and Elias on the preshow, which placed him in the United States Championship triple threat match (7.10)

— Daniel Bryan won the SmackDown Elimination Chamber for a title shot against Universal Champion Roman Reigns (34.20)

— WWE Universal Champion Roman Reigns defeated Daniel Bryan by submission (1.32)

— Matt Riddle defeated Bobby Lashley and John Morrison in a triple threat match to become the new United States Champion (8.40)

— Shayna Baszler and Nia Jax defeated Sasha Banks and Bianca Belair to retain the Women’s Tag Team Championship (9.35)

— Drew McIntyre defended the WWE Championship in the Elimination Chamber, defeating Sheamus, Kofi Kingston, Randy Orton, AJ Styles, and Jeff Hardy (31.10)

— The Miz cashed in the Money In The Bank case to defeated Drew McIntyre and become the new WWE Champion (0.30)

Apparently Paul Wight (Big Show) contract with WWE expired last month and couldn't come to a agreement with Vince, as he has signed with AEW he may be on commentary with Tazz and Jim Ross before stepping into the ring.
