It depends what you mean by 'substantially'. It's not like the difference between Apocalypse Now and Apocalypse Now: Redux.
However, the director's cut, from memory, has more dialogue, developing the relationship between Kinderman and Dyer and less footage of Jason Miller's Patient X. Legion has much more of Brad Dourifâs Gemini Killer (and is better for it), plus the beginning and ending of the film is different to the theatrical cut.
It's definitely worth watching both and deciding which one you feel is closest to the source material, William Friedkin's 1973 film, or you find most enjoyable/entertaining/thrilling.
