Top 10 Extreme Horror.

Zombie (1979)
Inside (2007)
Aftermath (1994)
Irreversible (2002)
The Life and Death of a Porno Gang (2009)
Bone Sickness (2004)
Hostel: Part II (2007)
Gutterballs (2008)
Scrapbook (2000)
Fantom Killer 3 (2003)

Fulci's undead classic may now be considered tame enough for an 18 UK release but i've always considered it to be the benchmark of quality gore thanks to the outstanding gut wrenching effects from Giannetto De Rossi. As a film it's my number one as it's the greatest zombie film ever!

Some great choices there, Dem - a few of which might make my list too.

It's great seeing how everyone is defining 'extreme horror' too as there are no real right or wrong answers with it being such a subjective term.

I'm glad to see a few of my likely inclusions turning up already.

Haven't seen much really that comfortably fits into the "extreme" category, a lot are on my watch list but usually they either don't appeal or I have other things I'd rather watch than someone getting brutally raped or tortured (brutally murdered on the other hand, now that's a different story).

1. Cannibal Holocaust
2. Ichi The Killer
3. Nekromantik
4. The New York Ripper
5. Audition

And not classic extreme but fit my bill
1. Lady Vengeance
2. Story of Riki-Oh
3. Evil Dead (Remake)
4. Maniac (Remake)
5. House By The Cemetery

In no particular order;

August Underground; Mordum. Just. Pure. Nasty.
Slaughtered Vomit Dolls. The name should say all you need/want to know.
Men Behind the Sun. T F Mous' masterpiece but a gruelling watch.
A Serbian Film. New born porn? 'Nuff said.
Flower of Flesh and Blood. The original WTF? film.
Africa Addio. The granddaddy of mondo.
Grotesque. No rhyme, no reason, no plot, just nasty.
Amerikan Holocaust. Masturbation with innards.
Shocking Africa - The circumcision one not the tame mondo you can see on Amazon etc.
Lucker the Necrophagus. Before Nekromantik there was Lucker.

Honourable mentions;
10 Monologues of the Lives of the Serial Killers. Disturbing and one of the few films I've only ever watched once.
Dead Man 2; Return of the Dead Man. Made me physically gag! True.
Aftermath. Morgue mayhem.
Forced Entry. "Is it starting to bleed a bit in there, lady?"
El Gore's Snuff Tape Anthology. FFS!!!
Waterpower. Why? Just why??
Adam Chaplin. More blood than you can shake a stick at.
Salo; 120 Days of Sodom. Hmmm....dinnertime!
Richard Kern's Hardcore Collection. Lydia Lunch, Fingered. Pure gold.
Entrails of a Virgin. Masturbation with a severed arm.

You surprise me with this one, Boo.

I thought it really dull.

It was a time and place thing for me. When I first saw it, I had never seen anything like it and it always stayed with me as one of those extreme type of films. And you're right, it is a bit dull, also inept, badly done in every aspect, but holds a place in my memory for it's impact on a young Boo.
Available on DVD from Germany.
All my list is available on disc except 10 Monologues and Dead Man 2, which I have on VHS, and Waterpower, which can be found if you look around DVD-r sites. It was on Amazon but is currently unavailable....

My version of Lucker is American. Probably cut.

OK - extreme horror. Something I can comment on!

1: Cannibal Holocaust - Of course this is going to be top of the list. It's true that films that are more extreme have been made, but this one feels so real
2: Freaks - It's an oldie, but a goodie. It's one of the few true horror films ever made
3: Combat Shock - It's nasty, and probably a 'one watch' film for me! For once, believe everything you have ever read or been told about this film.
4: Henry: Portrait Of A Serial Killer - Supremely well made, and like 'Cannibal Holocaust' that is the problem. It is too believable
5: Centipede Horror - Sickening Japanese film, with genuine footage of women vomiting up live centipedes. Yuk!
6: The House On The Edge Of The Park - The mixture of sex and violence makes this an endurance for me to watch. Again, David Hess is just too believable here
7: Blood Feast - Has to be included for historical purposes
8/9: Red Room/Eating Schoolgirls: Osaka Telephone Club - More J-Horror. I have only seen these two once, but I remember then both as being nasty!
10: The Evil Dead - Some fantasy to end on!

1. The Last House on Dead End Street (1973)
2. Tetsuo: The Iron Man (1989)
3. Visitor Q (2001)
4. Subconscious Cruelty (2000)
5. Nekromantik (1987)
6. The Life and Death of a Porno Gang (2009)
7. Begotten (1991)
8. Salò, or the 120 Days of Sodom (1975)
9. Gutterballs (2008)
10. Bone Sickness (2004)
